Is PA/LTC a Team Sport?

In California, Post-Acute and Long Term Care is rapidly changing with emphasis on improving the patients experience of care and on providing high value care.  The latter demands better care at lesser cost.  At CALTCM, we now have experience achieving this goal through our partnership with INTERACT to provide California-specific training.  We have provided this training in partnership with hospital / SNF partners.  We now have data documenting a significant improvement in 30-day readmission rates and other quality metrics.  This data will be on display at the POSTER session of our annual meeting in May. 

Our California INTERACT training program encourages staff at PA/LTC facilities to operate as a "team".   We have learned that facility Action Plans are the road map that set the expectations and future accountability for next steps in a quality improvement project.  They also provide the transparency so helpful in working with hospital partners.

Our annual meeting this year models what we have learned in our INTERACT work.  I have attached the proposed Action Plan worksheet for our first of 3 workshops at this year’s annual meeting.  As you can see, we are serious about partnering with you to provide the best care for your patients at a value that is affordable.

There is still time to send a team to our annual meeting to take advantage of workshops designed to insure your facilities success.
