CALTCM Lifetime Achievement Award: Gieseke

Celebrating a Career of Superlative Leadership, Outstanding Achievements, and Visionary Contributions to Long Term Care

Timothy L. Gieseke, MD, CMD

Dr. Timothy Gieseke is a general internist who has been a member of the Santa Rosa Medical community since 1979. Over the first 15 years, his practice transitioned to focusing on the care of elders living in the community and in congregate living settings.

He became the Medical Director of Spring Lake Village, a large Continuing Care Retirement Community (CCRC), in 1986 and became a certified Medical Director for SNFs in 1996. About that time, he became the Medical Director for Santa Rosa Memorial’s two Subacute Units. In that role, he was a member of this hospital’s Medical Executive committee for about 7 years. As those units were closed by Memorial, he became their Medical Director of Utilization Review. In 2005, he closed his office practice, and worked full time in the SNF and ALF settings. This allowed him to become an associate Medical Director in 2006 for Sutter Hospice and become more involved in AMDA and CALTCM. He became President of CALTCM (2007 and 2008), was the Education Chair (2010-2011 and 2014-2015), Treasurer (2016-2019), and led the search committee for a new CEO in 2020. He began contributing articles to the WAVE in 2013 and found great joy sharing opportunities to improve clinical care. He taught post-acute and LTC medicine at the local family medicine residency as an Associate Clinical Professor for UCSF (2010-2019). He chaired red eye rounds at the American Medical Directors annual meeting (2016-2019). He also was privileged to be a part of multiple short-term medical education projects in Ecuador (1990), Albania (1992-2013), Kosova (2008, 2009), and Cuba (May 2023).
After retiring at the end of 2019, he went back to work in March 2020 as volunteer leading Sonoma County’s “Learning Collaborative” for mitigating the impact of COVID on the PALTC communities. Fox Home Health where he had continued as their Medical Director, contributed to efforts to educate and vaccinate hard to reach people groups. He became a consultant for two local CCRCs to enhance their COVID mitigation efforts and provided locum tenens physician illness and vacation coverage for the larger CCRC. He continued as CALTCM’s representative to the California Partnership to Improve Dementia Care and supported many of their pandemic related teaching initiatives. In 2021, he became the Medical Director for a new hospice targeting residents of small CCRCs.
As the pandemic improved by the end of 2023, he was asked to provide maternity coverage for a local PACE program and thoroughly enjoyed their team and targeted approach for improving the lives of poorly funded frail participants. When that was completed, he was able to provide post-acute rehab services for the Medical Director of Spring Lake Village until a much younger colleague with expertise was identified. This person also assumed his other medical director positions permitting retirement again as of March 2026.
