Homecoming Weekend for CALTCM

I look forward to family reunions.  It’s great to be with people who understand you, share common interests, and are committed to your success. 

CALTCM is our family.  It’s a safe place to share our passion, to develop close friendships, and to develop professionally in a stimulating environment.  We understand each other’s work and know the challenges of translating best practices into best care practices at a team and facility level.

As chair of our education committee, I am proud of this year’s annual meeting.  Our committee has worked hard to have this be a practical engaging meeting in which your problems are addressed.  As an innovative organization, we will provide learning around the table for each of our 3 workshops.  In addition, we expect you to leave our meeting with practical next steps for improving the care your patients experience in your facilities and your communities.  As we become part of integrated care systems, this partnering will be crucial for the success of each of our facilities.

Please see the course brochure below.  We have integrated QAPI (Quality Assurance Performance Improvement) into each of our 3 workshops.  You have told us that these clinical problems are important.  Let’s write a new success story for:

  1. Challenging wounds
  2. Dementia problem behaviors
  3. COPD readmissions and integrated care models

Together, we can make progress in these challenging areas. 

So my friends, homecoming is rapidly approaching.  Your family is calling.
